Authentication Instructions and online information form

1. Try to answer all questions in Parts I and II of the Authentication Form (available online -- see form below); information should come from your own knowledge. If data comes from a document or from another person, put that information in "quotation marks". Please do not guess.
2. You should complete a separate Authetication Form online for each of your prints, except for sets (portfolios) of prints.
3. Photographs: We require three photographs of your print. You can upload these photos below with the authentication form submission or you can email them to us. The three photos should show:
   
   1. the complete image;
   2. the signature (close-up), and
   3. the numbering (close-up).
4. If you have any relevant documents, such as Certificates of Authenticity, Invoices, etc, you can either scan these items and upload them in the form below or make copies of them and send them to our mailing address.
5. Please enclose a check or money order for the fee. We do not accept credit cards.

Checklist of materials you need to send the Archives:

1. Complete the online Authentication Information Form below.
2. Upload the (3) photographs of your print, as specified above, or send them via E-mail.
3. Upload any scanned documents related to your print in the form below or send copies via postal mail.
4. Send us a check or money order for the authentication fee relative to the work(s) you are submitting.

NOTE: Please allow six (6) to eight (8) weeks for your report to arrive.
